Emergency Certificate
1. Purpose
This document is issued to Malaysian Citizens who have lost their passports while abroad, who have been repatriated to Malaysia at the Government’s expense and to those whose Malaysian Passport cannot be issued within the stipulated period.
2. Validity
Valid for a Single Journey to countries mentioned therein for the period of 12 months
3. Conditions
- Form F- Im.42-pin 1/97
- Payment of RM50.00 per person
- Documentary evidence of Malaysian status
- Two (2) photographs of applicant (3.5 cm x 5.0 cm )
