2.Borang Imm.38

3.Statutory Declaration by the husband and wife that the marriage is still binding.

4.Children’s birth certificates (if any)

5.Identity cards of both husband / wife

6.Marriage certificate (abroad / India)

7.Passports of both husband / wife (all printed pages)

Note:
• Husband and wife must be present
• For the nationals of China and African countries, the completed application form must be submitted to the Malaysian Immigration Department Headquarters.
