Loss of Passport

 

Letter from the employer stating how the passport is lost

Certified Police Report

Photocopy of Lost passport

Receipt of Deposit/Photocopy of Bank Guarantee/Photocopy of Insurance Guarantee (excluding domestic helper)

Photocopy of new passport

Other documents related to the case of the lost passport

Upon approval of the application, the employer should:

o Produce the original new passport

o Pay for the transfer endorsement fee

o New sticker of VPTE is issued in the new passport

 

In cases where the application is rejected, the employer should make the arrangement for the worker to be sent back to the country of origin.

 

Arrested Foreign Worker

 

In cases where the foreign worker was arrested, the employer should produce the document as stated below to the Immigration Department for further action:

 

Letter from the employer

Letter from Police Station or Immigration Depot

Original and photocopy of the worker's passport

 

Temporary Transfer of Workers in Construction and Services Sector (Cleaning Services) to the

 

New Location

 

Employer can submit application to transfer the workers in construction and services sector temporarily to the new location. The documents required are:

 

Letter from the employer

Letter of Contract / Letter of Award

Name List of foreign workers involved

Photocopy of worker's passport

 

Change of Company's Name and Address

 

The documents required are:

 

Letter from the employer

Company's Registration Form (Form 49 / Form D)

Form 13 (if applicable)

Telephone and Utilities bill

Tenancy Agreement - for rented premises

Name List of foreign workers involved

Photocopy of worker's passport

 

Refund of Levy

 

Refund of levy can be made by the following conditions:

 

Letter from the employer

Original payment receipt or declaration form for lost of receipt

Levy Refund Form

 

Refund of Deposit

 

Refund can be made only when the worker had left Malaysia. The documents to be produced:

 

Letter from the employer

Original payment receipt

Check Out Memo

Company's bank account or Identification Card (for Individual Employer)

 

Release of Bank Guarantee / Insurance Guarantee

The release of Bank Guarantee / Insurance Guarantee can be made only when the worker had left Malaysia. The documents to be produced:

 

Letter from the employer

Photocopy of Bank Guarantee / Insurance Guarantee & Name List of Foreign Worker

Check Out Memo

Death Certificate of worker
