  Purpose
    		• This document is issued to Malaysian Citizen under circumstances such as in cases of lost passport abroad, persons who have been repatriated to Malaysia on Government expenses and in cases where passport cannot be issued within the stipulated period.
     
      Validity
  • Valid for Single Journey to countries mentioned therein for the period of 12 months
    •

    		• Payment of RM50.00 per person
    		• Documentary evidence of Malaysian status
    		• Two (2) photographs of applicant measuring 3.5 cm x 5.0 cm
