  Malaysia Restricted Passport To Singapore

  Purpose
  		• This Passport is issued to Malaysian citizen for the purpose of travelling between West Malaysia and Singapore.
     
      Validity
  		• Five (5) years from the date of issue
     
      Conditions
  		• Form A1/B1 (father's permission if relevant)
  		• 2 photographs of applicant measuring 3.5 cm x 5 cm
  		• Form D- Im.50
  		• Old Restricted Passport (if any)
  		• Payment of RM 150 for five (5) years

  		• Statutory Declaration of Guardianship
    Below 18 years Parents Separated (Father cannot be contacted)
  		• Identity Card (if any)
  		• Birth Certificate
  		• Mother's Identity Card
  		• Court's Order
  		• Statutory Declaration by mother
  		• Declaration by mother whose father cannot be contacted
    Below 18 years Adopted Child
  		• Identity Card (if any)
  		• Identity Card of Legal Guardian
  		• Certificate of Adoption certifying child's citizenship status
     
      Application for Renewal of Malaysia Restricted Passport
  		• The validity of Malaysia Restricted Passport is five (5) years from the date of issue unless stated otherwise. The passport may be renewed for another five (5) years.
  		• Conditions:
  • Form D-Im.50B
  • Malaysia Restricted Passport
  • Identity Card (if any)
  • Birth Certificate (if below 12 years old)
  • Adoption Certificate (if relevant)
  • Photocopies of Identity Card, Birth Certificate, Adoption Certificate
  • Payment : RM30 per year
